What patients do in the first 24-72 hours after injection can affect filler migration, swelling severity, bruising, and even how long the results last. Proper aftercare is simple but specific.

The First 24 Hours: Critical Instructions

What to Expect Immediately After

  • Swelling at the injection sites, which can make the area look over-filled. This is normal.
  • Redness and small bumps at needle entry points
  • Tenderness when touching the treated area
  • Minor bruising at some injection sites
  • The treated area may feel firm or lumpy. This is the filler before it integrates.

The Do Not Touch Rule

Do not press, massage, or manipulate the treated area for 24 hours unless your injector specifically instructs otherwise. Pressure can displace the filler before it has settled into position. This means:

  • No alcohol for 24 hours. Alcohol dilates blood vessels and increases bruising.
  • No blood-thinning supplements (fish oil, vitamin E, ginkgo biloba) for 24 hours before and after.
  • No extreme heat: saunas, hot yoga, steam rooms, sunbathing. Heat causes the hyaluronic acid to break down faster and increases swelling.
  • No extreme cold: do not apply ice directly to filler areas (particularly lips). Brief cold compresses are okay for bruising.
  • No makeup on the injection sites for 12-24 hours to prevent infection.

Days 2-7: Swelling and Settling

The Swelling Timeline

Filler swelling follows a predictable pattern:

  • Day 1: Moderate to significant swelling. The area looks over-done. This is the filler plus inflammation.
  • Day 2-3: Peak swelling for most patients. Lips especially can look very swollen.
  • Day 3-5: Swelling begins to decrease noticeably.
  • Day 7: Most swelling has resolved. You can start to see the true result.
  • Day 14: The filler has fully settled. This is when the final result is assessed.

Swelling is worse in the morning (fluid pools in the face overnight) and improves throughout the day. Sleeping with an extra pillow to elevate the head helps.

Bruising Management

About 20-30% of patients experience some bruising. Management:

  • Arnica (oral or topical) can reduce bruising severity
  • Vitamin K cream applied to bruised areas
  • Avoid blood thinners (aspirin, ibuprofen, fish oil)
  • Bruises typically resolve in 7-14 days
  • Concealer can be applied after 24 hours

When the Filler "Settles"

Over the first 1-2 weeks, the filler hydrates (attracts water) and integrates with the surrounding tissue. During this time:

  • The area will soften and feel more natural
  • Any asymmetry from swelling will resolve
  • Small lumps typically smooth out on their own
  • The filler may appear to "shrink" slightly as swelling resolves - this is normal, the filler is still there

Area-Specific Aftercare Tips

Lips

Lip filler swelling is the most dramatic. Expect the lips to look significantly over-filled for 3-5 days. Tips:

  • Drink through a straw for the first 24 hours (less movement of the lips)
  • Avoid very hot or very spicy foods for 48 hours
  • Use a gentle lip balm to keep the lips moisturized
  • Avoid kissing with pressure for 48 hours
  • Numbness or tingling is normal and resolves in hours

Cheeks and Midface

  • Sleep on your back for 2-3 nights
  • Be cautious with sunglasses that sit on the treated area
  • Swelling here is often less obvious than with lips

Jawline and Chin

  • Avoid resting chin on hands
  • Be gentle when shaving the jawline area
  • The result may feel firm initially - this softens over 2 weeks

Under-Eyes (Tear Trough)

  • This is the area most prone to visible swelling
  • Do not touch or massage under the eyes
  • Elevate head while sleeping for at least 1 week
  • Avoid salty foods (increases puffiness)
  • Final result may take 3-4 weeks to appear

Nasolabial Folds and Marionette Lines

  • Be careful with dental work for 2 weeks
  • Avoid exaggerated facial expressions for 48 hours
  • These areas typically settle quickly

Long-Term Care and Maintenance

How Long Fillers Last

Duration varies by product and area:

  • Lips: 6-12 months
  • Cheeks: 12-18 months
  • Jawline and chin: 12-24 months
  • Under-eyes: 12-18 months
  • Nasolabial folds: 6-12 months

Regular touch-up appointments (every 6-12 months) maintain the result. Some studies suggest that regular filler maintenance can stimulate natural collagen production, meaning you may need less product over time.

Skin Care

After the initial 2-week settling period, there are no restrictions on skincare products. However, maintaining good skin health with retinoids, sunscreen, and antioxidants helps the filler look better and can complement the volumizing effect.

Signs of Complications

Seek Immediate Attention

  • Skin turning white, grey, or dusky blue near the injection site (vascular occlusion - this is urgent)
  • Severe pain that worsens over 24-48 hours
  • Vision changes after treatment near the nose or forehead (vascular occlusion - emergency)
  • Fever combined with increasing redness and warmth
  • Pus or unusual discharge from injection sites
