Facelift Aftercare: The Complete Recovery Guide for Best Results

Published on February 24, 20267 min read

Why Facelift Aftercare Is Critical for Natural-Looking Results

A facelift (rhytidectomy) is one of the most transformative cosmetic procedures available, but the quality of the result depends heavily on what happens during recovery. The surgical technique creates the foundation, but proper healing determines how natural, smooth, and long-lasting the outcome will be.

For plastic surgery clinics, facelift patients tend to be highly motivated and detail-oriented. They invest significantly in the procedure and expect comprehensive aftercare guidance. Delivering structured, timely aftercare instructions builds trust, reduces complications, and generates the kind of referrals that grow a practice.

The First 48 Hours: Drains, Dressings, and Positioning

Immediate Post-Operative State

Patients wake from anesthesia with a compression dressing wrapped around the head and under the chin. Small drain tubes may be placed behind the ears to prevent fluid accumulation. The face will feel tight, numb, and swollen.

Common experiences in the first 48 hours:

  • Tightness across the face, neck, and behind the ears
  • Numbness in the cheeks and jawline (this is normal and temporary)
  • Swelling concentrated around the jawline, neck, and ears
  • Bruising that extends from the ears down the neck and sometimes onto the chest
  • Difficulty opening the mouth wide (eat soft foods)
  • Mild nausea from anesthesia

Head Elevation

Keep the head elevated at 30-45 degrees at all times for the first 2 weeks. This is the most important instruction for controlling swelling. Use a wedge pillow or recliner. Do not lie flat, even for naps.

Drain Care

If drains are placed, patients need to:

  • Empty the drain bulbs 2-3 times daily and record the output
  • Keep the drain sites clean and dry
  • Report if output suddenly increases or becomes bright red
  • Drains are typically removed at 1-3 days post-op

Pain and Medication

Facelift pain is generally moderate. Most patients describe it as tightness and soreness rather than sharp pain. Take prescribed medications on schedule. Avoid aspirin, ibuprofen, and blood-thinning supplements for at least 2 weeks.

Week 1: The Critical Healing Phase

Dressing Changes

The compression dressing is typically removed at 24-48 hours and replaced with a lighter wrap or chin strap. The surgeon will check the incisions, remove any sutures that are ready, and assess healing.

Hair Washing

Most surgeons allow gentle hair washing starting 48-72 hours post-op. Use lukewarm water, baby shampoo, and very gentle pressure. Do not scrub near the incision lines. Pat dry, do not use a blow dryer on hot settings near the incisions.

Incision Care

Facelift incisions typically run in front of and behind the ears, sometimes extending into the hairline. Care involves:

  • Keeping incisions clean with gentle cleansing as directed
  • Applying prescribed ointment or silicone strips
  • Avoiding any pulling or tension on the incision lines
  • No earrings for 4-6 weeks

What to Eat

Stick to soft foods for the first week: soups, smoothies, yogurt, mashed potatoes, scrambled eggs. Chewing is uncomfortable due to the tightness and swelling around the jaw. Avoid very salty foods as they increase fluid retention and swelling.

Weeks 2-4: Visible Recovery

Swelling and Bruising Timeline

  • Week 1: Maximum swelling and bruising. The face looks distorted.
  • Week 2: Bruising begins to fade. Swelling decreases noticeably. Most sutures or staples are removed.
  • Week 3: Most visible bruising has resolved. Swelling is improving but still present, especially along the jawline and neck.
  • Week 4: About 70-80% of swelling has resolved. Most patients feel comfortable being seen in public with minimal concealer.

Return to Social Life

Most patients feel presentable for social situations at 2-3 weeks, though some residual swelling and bruising may be visible. Concealer makeup can be applied to bruised areas once all incisions are fully closed (typically around day 10-14).

Activity Restrictions

  • Walking: Encouraged from day 2-3, starting gently
  • No bending over or heavy lifting for 3-4 weeks
  • No vigorous exercise for 4-6 weeks
  • No swimming for 4-6 weeks
  • Sleep on your back with head elevated for a minimum of 3 weeks

Skin Care

  • No retinoids, glycolic acid, or harsh actives near incisions for 4-6 weeks
  • Gentle moisturizer is fine on non-incision areas
  • SPF 50+ sunscreen daily once skin has healed. Sun exposure can permanently darken scars and worsen swelling
  • No facial treatments (microdermabrasion, laser, peels) for 3-6 months

Months 1-3: Continued Refinement

Numbness and Sensation

Facial numbness after a facelift is entirely normal and resolves gradually over 3-6 months, sometimes longer. Patients may experience:

  • Decreased sensation in the cheeks and jawline
  • Tingling or "pins and needles" as nerves regenerate
  • Tight or "pulling" sensations, especially when smiling or turning the head
  • Areas of the ear that feel numb

Reassure patients that sensation returns in most cases, though it may take up to 12 months for full recovery.

Scar Maturation

Facelift scars are typically well-concealed in the natural creases around the ears and hairline. They evolve over 12-18 months:

  • Months 1-3: Red, possibly raised
  • Months 3-6: Gradually fading and flattening
  • Months 6-18: Continuing to lighten and blend with surrounding skin

Scar care includes silicone gel or strips, gentle massage (starting at 3-4 weeks), and strict sun protection.

Returning to Full Activity

By 6-8 weeks, most restrictions are lifted. Patients can resume:

  • All forms of exercise
  • Facial treatments and skincare actives (check with surgeon)
  • Sleeping in any position
  • Wearing earrings

Long-Term Results and Maintenance

How Long Does a Facelift Last?

A well-performed facelift typically provides results that last 7-10 years. The face continues to age naturally, but from a more youthful starting point. Factors that affect longevity:

  • Sun protection: UV damage is the #1 accelerator of facial aging
  • Smoking: Dramatically accelerates skin aging and compromises healing
  • Weight fluctuations: Significant weight changes affect facial volume
  • Skincare routine: Retinoids, antioxidants, and SPF help maintain results
  • Non-surgical maintenance: Many patients use Botox, fillers, or laser treatments to extend their facelift results. Some also consider complementary procedures such as rhinoplasty or eyelid surgery for comprehensive facial rejuvenation

Signs of Complications

Contact the surgeon immediately if you notice:

  • Sudden, severe swelling or pain on one side of the face (may indicate hematoma, the most common serious complication)
  • Bleeding that soaks through the dressing and does not stop
  • Fever above 38.5C
  • Skin that turns dark, purple, or white (may indicate compromised blood supply)
  • Increasing redness or warmth along incision lines
  • Foul-smelling drainage from any incision
  • Difficulty breathing or a feeling of tightness in the throat

Hematoma (blood collection under the skin) is the most common complication requiring intervention, occurring in about 2-4% of facelift patients. It usually presents as sudden, asymmetric swelling and pain within the first 24 hours and requires prompt surgical drainage.
